AFFIDAVITS

REQUIREMENTS

  • Dominican identity card.
  • Valid Dominican passport.
  • Recent Dominican Birth Certificate.
  • Identity Card of the DECLARANT of the country where he/she is.
  • Precise information of what the citizen pursues with the Affidavit.
  • A witness (depending on the type of declaration).

CONDITIONS

  • In case of a damaged passport, you must bring a damaged Dominican passport.
  • In case of an Affidavit of Marital Status and Address, precise information of the marital status and where the citizen has resided and currently resides.
