AFFIDAVITS
REQUIREMENTS
- Dominican identity card.
- Valid Dominican passport.
- Recent Dominican Birth Certificate.
- Identity Card of the DECLARANT of the country where he/she is.
- Precise information of what the citizen pursues with the Affidavit.
- A witness (depending on the type of declaration).
CONDITIONS
- In case of a damaged passport, you must bring a damaged Dominican passport.
- In case of an Affidavit of Marital Status and Address, precise information of the marital status and where the citizen has resided and currently resides.
