REQUIREMENTS

• Recently legalized Dominican birth certificate. (year 2014 to date)
• Dominican Identity and Electoral Card of the holder (New identity). In case it is for a minor, the identity card of one of the parents and a signed authorization of the same.
• Two (2) 2 “x2” photos, with the following characteristics:
  
  
  • Dull
  • White background
  • Front view
  • No glasses

  • No clothing (NO earrings, piercings, chains, cords, necklaces, etc.)

  • No ornaments (NO headbands, headbands, headgear, hats, caps, etc.)

  • No shadow behind you

  • No facial expressions (NO smiling, grimacing, no teeth, etc.).

  • With hair pulled back, forehead visible and ears clearly visible.

  • Babies with eyes open and mouth closed. 

• If lost, you must request an Affidavit of Loss.
• If you do not have the birth certificate, you can request it at the JCE office located at the Consular Section.
• Expired passport booklet.

• ROUTE CARDS

-The issuance of a provisional passport or letter of route is issued in exceptional cases to all Dominican citizens who do not have a valid passport and need to travel to the Dominican Republic. This document is issued for a validity of 30 days only and exclusively to return to the Dominican Republic. In order to be able to return to the country where the provisional passport was issued, the Dominican citizen must obtain the passport in Dominican territory.
